What is Refractive Lens Exchange?
Refractive Lens Exchange (RLE) is a vision correction procedure in which the eye's natural lens is replaced with an artificial intraocular lens.
The technique is similar to cataract surgery, but is primarily performed to correct vision before a visually significant cataract develops.
Modern lens technologies can be selected to address long-sightedness, short-sightedness, astigmatism and age-related difficulty focusing on near objects.

Could refractive lens exchange be right for you?
If you are over 50 and finding yourself increasingly reliant on reading glasses, RLE may be an option worth considering.
It may also be considered by patients with significant prescriptions who wish to explore alternatives to glasses or contact lenses.
However, RLE is not appropriate for everyone. Your prescription, retinal health, corneal condition, lifestyle and personal expectations all influence the most suitable approach.
Choosing the right lens for you
There is no one-size-fits-all intraocular lens. Your lens selection is guided by your eye measurements, prescription, lifestyle and visual expectations.
Monofocal Lenses
Designed to provide clear vision at a selected focal distance. Glasses are generally needed for other distances.
Toric Lenses
Designed to correct regular astigmatism, potentially improving uncorrected vision at the selected focal distance.
Extended Depth of Focus
Designed to provide an extended range of functional vision, particularly for distance and intermediate activities.
Multifocal & Trifocal
Designed to support vision across multiple distances and potentially reduce reliance on reading glasses.
Every lens has advantages and limitations. Premium lenses may cause halos, glare or reduced contrast sensitivity, and complete freedom from glasses cannot be guaranteed. These trade-offs will be discussed during your personalised consultation.
What happens during RLE surgery?
A carefully planned procedure with precision, comfort and safety at the centre of every step.
Your personalised assessment
A comprehensive eye examination and detailed measurements are performed to assess suitability and calculate the appropriate intraocular lens power.
Preparing your eye
The procedure is usually performed as day-case surgery under local anaesthesia. Anaesthetic eye drops help keep you comfortable.
Removing the natural lens
A small incision is created and specialised ultrasound technology is usually used to remove the natural lens while preserving its supporting capsule.
Implanting your new lens
Your carefully selected artificial intraocular lens is inserted and positioned, usually within the natural lens capsule.
Recovery and aftercare
Following surgery, you will receive instructions about eye drops, precautions and follow-up appointments. Vision generally improves as the eye heals, although recovery varies between patients.

Frequently asked questions
Understanding your options is an important part of making a confident, informed treatment decision.
Is refractive lens exchange the same as cataract surgery?
The surgical technique is very similar. Both procedures replace the natural lens with an artificial intraocular lens. However, RLE is performed primarily to correct vision rather than treat a visually significant cataract.
Will I be completely free from glasses?
RLE can significantly reduce reliance on glasses in appropriately selected patients, particularly with certain lens technologies. However, complete spectacle independence cannot be guaranteed, and some patients continue to need glasses for particular activities.
Is RLE suitable for everyone over 50?
No. Although RLE is commonly considered in patients experiencing age-related changes in near vision, suitability depends on your prescription, eye health and individual risk factors. Other treatments, including laser vision correction or continued spectacle use, may be more appropriate.
Is refractive lens exchange permanent?
The artificial lens is generally designed to remain inside the eye permanently. It does not develop a cataract. However, the capsule behind the lens can sometimes become cloudy, which may require YAG laser capsulotomy. Further treatments may occasionally be needed.
What are the risks of RLE surgery?
RLE is an intraocular surgical procedure and carries risks, including infection, inflammation, retinal detachment, macular oedema, visual disturbances and, rarely, permanent loss of vision. The risk of retinal detachment is particularly important in highly myopic eyes. Mr Hussien will discuss your individual risks and alternatives before any decision to proceed.
How long does recovery take?
Many patients notice visual improvement within the first few days, although clarity may fluctuate while the eye heals. Visual adaptation, particularly with multifocal lenses, may take longer. Your postoperative plan will be tailored to your recovery.
Can both eyes be treated?
RLE may be performed on both eyes when clinically appropriate. Treatment is usually planned as separate procedures, although the precise timing is decided following individual assessment and discussion.
